Roland Deschain and his ka-tet (Eddie, Susannah, Jake, and Oy) find themselves near the town of Calla Bryn Sturgis. The people of the Calla desperately need the gunslingers' help. In exchange, Father Callahan, a character from King's novel "Salem's Lot" who has found his way to Mid-World, offers Roland a powerful and dangerous seeing sphere called Black Thirteen.
The majority of the book focuses on the ka-tet's efforts to help the Calla prepare for the Wolves' return. This involves learning about the Wolves, understanding the unique culture of the Calla (where almost everyone is a twin), and devising a strategy to defend the town. Roland and his companions also grapple with personal issues and the ongoing quest for the Dark Tower, with connections being drawn to their past and future.
